Which step of the nursing process directly follows Diagnosis?

Assessment

Implementation

Planning

After a nursing diagnosis is made, the next step is planning. This phase sets goals and expected outcomes for the patient and selects appropriate interventions to address the identified problems, organizing them in order of priority and feasibility. Planning creates a roadmap for how care will be delivered to achieve desired results and guides what you’ll implement. Assessment, which happens before diagnosing, isn’t the next step. Implementation comes after planning, putting the planned actions into action, while evaluation occurs later to determine whether the goals were met and to adjust the plan if needed.
